All Medical Assessments
“All Medical Assessments” include Pre-employment medicals, Management Referral assessments to establish medical fitness for work, Ionising Radiation Medicals, Night Worker’s health assessments, Asbestos medicals (licenced and non-licenced), Assessments to issue advice on ill health retirement and health surveillance escalations (including but not limited to T4 HAVS assessment).
Cases are only seen on a Referral basis.
Managers can submit a referral form at the time of online booking or by getting directly in contact with us.
All medical assessments should only be booked by the manager once the referring manager obtained the employee’s consent for the referral and its content.
We offer paper-based, telephone/video and face to face assessments. These are specified at the time of booking. We reserve the right to change the format of the assessment if clinically indicated.
Face to face consultations take place at Wollaton Vale Health Centre, Wollaton Vale, Nottingham, NG8 2GR.
Under certain circumstances such as specificities of the case, scheduling difficulties or government restrictions (due to COVID-19 or other) remote consultations (via MS Teams) can be offered.
Consultation times vary. Please make all the necessary arrangements (time off work, etc.) and allow up to 1 hour for Management Referral consultations and up to 30 minutes for asbestos/ionising radiation medicals/night worker’s health assessment (excluding travel time, if applicable).
Sereg OccMed Ltd. aspires to offer a high quality and bespoke service, for that reason advice will only be issued on cases that have been referred by a manager and assessed by a suitably qualified clinician.
Reports/Health Records/Certificate of examination
Reports/Health Records/Certificate of examination is usually issued on the day of the appointment and the latest within 3 working days. The release of full Management Referral reports is subject to receiving consent from the employee. The completion of Health Records is subject to all necessary information made available for the Appointed Doctor (templates available).
Follow up and assistance
Most assessments can be concluded during one appointment, however, occasionally there is a clinical reason to advise a follow-up appointment for the employee. No follow-ups are included in any bookings. If follow-up is recommended for the employee, this can be arranged as a new appointment booking.
A complimentary telephone call up to 15 minutes is included for Management Referrals, if further clarification is required regarding the content of the report. If a more complex discussion is required regarding the case with the participation of the manager, HR, +/- the employee and their representative, a separate “Case conference” booking is required.
Payment
Full payment of fees can be made at the time of booking or alternatively an invoice will be issued dated the date of the booking and payments via bank transfer/card/PayPay must be made within a 30 day timeframe. Delay or failure may result in the appointment being cancelled and/or legal action.
Appointment date and time will be confirmed once payment has been received or alternative payment method agreed.
Booking is regarded as acceptance of these terms and conditions.
Cancellations and late arrivals
Appointments for all medical assessments can be cancelled or rescheduled.
If an appointment is rescheduled 48 hours or more in advance of the appointment time, there will not be any additional charge.
If an appointment is cancelled 48 hours or more in advance of the appointment time, appointment fees will be refunded, minus 10% of the fee paid at the time of booking, to cover administrative expenses.
If less than 48 hours’ notice of cancellation is given or the employee fails to attend the appointment (regardless of whether telephone/video/face-to-face), the full booking fee will be applicable. If the fee had been paid at the time of booking, refund will not be given and if the appointment had been invoiced, the invoice will remain payable.
If the employee arrives late or need to leave before the appointment is completed, the consultation may need to be shortened and the assessment may be incomplete. Employees arriving after the first third of their allocated appointment time will not be seen, unless there is a cancellation slot on the same day. Any fees paid at the time of booking will not be refunded/outstanding invoices will remain payable.
If less than 48 hours’ notice is given when rescheduling an appointment, Sereg OccMed Ltd. reserve the right to cancel the appointment. The fees paid at the time of booking will not be refunded/outstanding invoices will remain payable.
Appointment times or dates may need to be altered because of circumstances beyond Sereg OccMed Ltd.’s control. If this happens, I will contact you as soon as possible to make alternative arrangements. The appointment will be rescheduled to a time of your convenience. If it is not possible to reschedule the appointment to a time of your convenience a full refund will be issued.
Sereg OccMed Ltd. or its employees will not be liable for any costs you may have incurred in preparation for your appointment.
